Hours : Create your own schedule, Monday–Sunday, 8:00 AM – 8:00 PM (Flexible weekend hours available)

Job Title : HouseCalls Nurse Practitioner (NP)

Hours : Create your own schedule, Monday–Sunday, 8:00 AM – 8:00 PM (Flexible weekend hours available)
Responsibilities
- Conduct in-home health assessments for Medicare Advantage members, including:
- Reviewing past medical history, systems, and medications.
- Performing physical exams (e.g., monofilament tests, urine dipsticks, foot exams).
- Educating members on disease processes and medication compliance.
- Document findings electronically during and after visits (completed within 48 hours).
- Communicate results to PCPs and identify opportunities for clinical referrals.
- Ensure compliance with HIPAA regulations.
- Licenses & Certifications:
- Active NP license (unencumbered).
- National Board Certification through AANP or ANCC (Family, Adult, or Geriatric specialties only).
- BLS/CPR certification.
- Experience:
- Minimum 6 months as a Nurse Practitioner (no new graduates).
- Preferred: experience in geriatrics, home healthcare, or related fields.
- Additional Requirements:
- Reliable transportation.
- Proficient with electronic medical records and internet use.
